Compensation : $10.50 / hr to $12.00 / hr

SUMMARY OF DUTIES :

An employee in this position ensures proper resident care is provided by the direct care partners to residents in a Home for Aged (HFA) Assisted Living and Memory Care home and is responsible for monitoring each resident€™s individualized service plan.

Employees of this job work under the supervision of the Administrator. This employee is a working supervisor and responsible for his / her shift duties and the supervision of the Direct Care Partner(s) during his / her shift.

ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES

The following is a representative list of duties and responsibilities associated with this position :

Resident care is not passive maintenance; an active approach will always be taken.

Help admit and discharge residents according to procedure.

Assist the resident with the activities of daily living while promoting independence

Follow individual resident support plan, flow sheet and group assignment. Use as a guideline for providing care. Assist all residents as required and / or requested

Observe residents for physical / cognitive changes. Report all changes to the Direct Care supervisor promptly, to include any abnormal behaviors

Respond to ALL emergency calls in a prompt manner

Check and record vital signs and weights, as assigned.

Assist with bowel and bladder needs, as per resident plan of care.

Provide for or assist residents with activities of daily living as needed, including bathing / showering according to schedule, oral care, toileting, dressing, etc.

Encourage residents to participate in scheduled activities; assist residents in getting to activities. Coordinate and assist with activities as directed.

Assist residents with transfers and ambulation as directed and ordered by the physician, using proper body mechanics. Provide transportation for residents within facility and, at times, accompany to appointments outside facility.

Respond to emergencies as needed within unit or as directed by supervisor.

Participate in the set up / clean up of dining room. As needed, wait on tables, bus tables, restock pantry, wash dishes, and light cleaning of the pantry area.

Keep environment safe for residents (hallways clear of equipment and floors free of debris and linen).

Requisition and change resident supplies as needed.

Maintain restraint free environment at all times. Support fall prevention programs for all residents in need.

REQUIREMENTS

An up-beat and engaging personality

Genuine desire to work with the elderly and / or disabled

Certified Nursing Assistant Certification helpful but not required

Capable of exerting 25 to 50 lbs of force to assist elderly in and out of wheelchairs

Preferably, Associates or College Degree; High school diploma / GED at a minimum

Able to follow written and oral instructions

Be a minimum of 18 years of age

Are you CPR and First Aid Trained and possess a valid certificate / card ?

Do you have experience and knowledge of medical terminology, medical records management, medical supplies and equipment?

EDUCATION & EXPERIENCE

High school or GED equivalent

CNA Certification and / or minimum 2 years in health related elder care environment.

Previous work experience as a Direct Care Partner and a Medication Passer

PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT

Work is performed in a residential community setting; possible exposure to unpleasant conditions such as cleaning of human waste and bodily fluids to maintain a sanitary environment for residents, visitors and staff of the community.

Speaking and hearing required to perform job responsibilities; reading / writing; understand directions and communicate verbally.

Specific vision abilities required include color, close and far vision acuity, depth perception and ability to adjust vision focus.

Employee must be in good physical health, and have the ability to climb, balance, kneel, crouch, reach, walk, sit, lift, grasp, perform repetitive motions involving bending, twisting, stooping, pushing, pulling, fingering, wrist motions and foot operations;

ability to stand for an extended period of time without significant rest period; possess the strength / stamina to lift more than 50lbs with assist.
