Job Summary :

The LPN - Case Coordinator role oversees the collaborative care of patients referred to the assigned CHI-Franciscan Health (CHI-FH) facility under the direction of a physician or ARNP.

Works with primary care providers (e.g. physicians, pharmacists, diagnostic imaging technicians, and physical / occupational therapists) to offer guided access to the range of orthopedic and neurological services that meet patient needs.

Manages patients and assess the needs and requirements for the best possible outcome consistent with the scope of licensure and CHI-FH standards / procedures.

Incumbents are also accountable for : 1) using effective communication, customer service and organizational skills in order to effectively oversee patient care.

2) using initiative in carrying out recurring tasks independently within scope of practice and established standards of care, 3) seeking direction from a Physician or ARNP for new specific instructions, difficult or unusual tasks, and when unusual / unfamiliar situations are encountered, and 4) maintaining an organized and safe work and patient environment, adhering to the hospital’s Safety, Risk Management and regulatory standards.

Essential Job Duties :

Guides the patient through the intake, diagnosis, treatment and discharge process in collaboration with other healthcare providers in the Spine Center team, the patient and family.

Greets and prepares patients for practitioner visit; obtains and records vital signs and other appropriate data (e.g. height, weight, drug allergies, current medications.

Etc.) and presenting problem(s); screens, communicates and responds to patients’ needs appropriately and within scope of practice.

Performs patient discharge activities following established procedures; follows up with patient after discharge as necessary.

Assures that exam room are turned around in a timely manner between patients.

Coordinates the timely flow of patients to allow adequate time for patient and family education.

Observes general and specific physical and behavioral signs and symptoms indicative of change in patient’s condition in accordance with established procedures and standards of care.

Recognizes implications of change, takes appropriate action within designated scope of responsibility and reports to ARNP or provider.

Advocates, on behalf of patients and caregivers, for service access and for the protection of the patient’s health, safety and rights;

advocates on behalf of the organization to promote safety, quality and appropriate resource utilization.

Communicates with patients, physicians, and outside agencies regarding the coordination of care and treatment of spine center patients;

handles incoming phone calls, pulls charts, receives and coordinates pharmacy re-fills and handles physical / occupational therapy re-certifications.

Qualifications

Education / Work Experience Requirements

Graduation from an accredited Practical Nursing program (or equivalent recognized military training), and two years of related clinical work experience that demonstrates attainment of the requisite job knowledge skills / abilities.

Case management work experience is strongly preferred.

Licensure / Certification :

Possession of a current Washington State license to practice as a Licensed Practical Nurse.

Current healthcare provider BLS certification.
