Provides case management for patients with chronic kidney disease. Collaborates with internal and external providers to coordinate the plan of care and provide expertise in chronic kidney disease treatment including dialysis. Accountable for ensuring effective coordination of patients with chronic kidney disease.Essential Responsibilities:Acts as case manager for the care and follow up of patients in the specialty clinic and outpatient dialysis centers. Arranges and monitors follow-up appointments. Encourages members to follow prescribed course of care (e.g., drug therapy, physical therapy). Makes referrals to appropriate community/contracted services.Collaborates with patient, patient support, and interdisciplinary team members in assessing patient's needs. Coordinates with other programs such as chronic condition management, specialty care, supportive care services and others to assure seamless transitions.Provides patient education and consultation for patient and family members throughout disease progression.Consults with internal and external physicians, health care providers, specialty care departments, discharge planning and outside agencies regarding continued care/treatment or hospitalization. Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning, and obtaining all authorizations/approvals as needed for outside services for patients/families.Provides expert consultation with physicians and other health care providers to enhance and monitor the quality and the level of care provided for patients affected by, or at-risk for, Chronic Kidney Disease/End Stage Renal Disease.Coordinates transmission of clinical and benefit information to patients, families and outside agencies. Assures regulations, health plan benefits, policies, and procedures are properly administered for members by medical office staff, contract providers, and outside agencies.Identifies service issues and implements systems changes, resulting in improved education and treatment of members. Provides on-going program planning and management, assists with development of region-wide work flow processes, and promotes standards of care for patients with, or at-risk, for Chronic Kidney Disease/End Stage Renal Disease.Directs and participates in the quality assurance and improvement activities, promoting cost-effective delivery of services, improving internal and external customer satisfaction, and ensuring positive patient outcomes. Identifies and promotes improved innovative clinical practice based on new knowledge and research in field of specialization, using a broad scope of nursing expertise.Maintains up-to-date knowledge of current activities in Nephrology and recommends/implements enhanced clinical practice standards for the department and the region. Ensures that accurate clinical information and data is maintained.Performs other Nephrology specific duties to support CKD and ESRD patient needs.Basic Qualifications:Experience
Minimum of two (2) years of dialysis and/or nephrology case management experience within the past seven (7) years.
Education
Bachelor's degree in nursing or health related field, OR four (4) years of experience in a directly related field.
Graduate of an accredited school of nursing.
High School Diploma or General Education Development (GED) required.
License, Certification, Registration
Current California RN License required.
Additional Requirements:
Knowledge of Nurse Practice Act, The Joint Commission and other federal/state/local regulations.
Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
Previous case management experience preferred.
Master's degree preferred, Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred, required to obtain within 2-years.
RCM Case manager experience.
