To identify and respond to the needs of the consumers

To assist the individual to reach the goals identified in the Treatment Plan

To provide emergency on-call coverage in rotation with other team members as needed. This may involve both phone work and in-person contact over a 24-hour period.

Maintains required records accurately, comprehensively and in a timely manner. Keeps required consumer data up to date and submits paperwork as per program policy.

Insures the confidentiality of all client and/or employee-related information.

Required Experience3 years preferred in social work or related fieldRequired EducationBA/BS in Human Service FieldRequired QualificationsValid Iowa Drivers License Successful completion of criminal background check, DHS Abuse Registry and Driving recordProgram SummaryAssertive Community Treatment is a multi-faceted team approach to assisting clients with persistent and severe mental health diagnosis to live in the community.
