We are seeking caring and compassionate individuals for the role of Direct Care Professionals, who are committed to building meaningful relationships in order to provide outstanding care and service for our clients. We offer both Full-Time, Part-Time and On Call options at our residential and day programs in various shifts.Responsibilities/DutiesThe Direct Support Professional is responsible for completing daily tasks in the residence and the community necessary for ensuring the individuals in the program recieve the appropriate care as outlined by Individual Support Plans, medical orders for medication and treamtent and agency policies and procedures. The DSP communicates regularly with other members of the team and managers to ensure the highest quality of care is being provided. The DSP ensures individuals are provided with opportunities for choice, community participation, and skill development towards the goal of attaining the greatest independence possible.The Direct Support Professional (DSP) is responsible for: ADMINISTRATIVE:Attends and participates in meetings and training related to the services provided in the program (inlcuding but not limited to team meetings, ISP meetings, administrative meetings).Ensures program documentation (data sheets, shift notes, etc. ) are completed in a timely and accurate manner.Prepares ISP materials for submission to supervisor (monthly progress notes, assessments assigned by supervisor)Monitors and documents funds present in the home (client funds, petty cash)Orders Medication for the program and ensures an adequate supply of medication is on-hand at all times.
Communicates medical information and follow up
-Uses cell phone (company or personal with stipend) to respond to calls (staff, supervisors, doctors, families, DDS, etc.)
Ensures staff recieve regular trainings in fire safety and human rights.
Reads and answers email in a timely matter
INDIVIDUAL SERVICES:-Provides opportunities for individual choice and independence-Implements individual goals as outlined in Provider Support Strategies-Supports individuasl to participate in all scheduled programs-Advocates for the need of individuals in the program-Monitors medication/provides medical treatments as outlined in orders by medical providers-Ensures that all medical treatments are delivered according to written orders (medications treatments, diets etc.)-Communicates all important information with supervisors-Ensures that monthly fire drills are conducted, documented and meet EESP guidelinesTRAINING: -Participates in regularly scheduled team meetings-Attends all assigned trainings (in person and online in the Cornerstone system)PROGRAM SITE:-Ensures that programs are clean and safe, reports any safety issues to appropriate supervisors (Site Manger/Cluster Manager/Residential Director)Ensures the necessary program supplies are present (food, cleaning materials, materials needed for ISP goals etc.)DOCUMENTATION:-Accurately records all data related to ISP goals-Records all necessary medical/medication informationOther duties as assigned.Required ExperienceWe are Hiring Dynamic Team Members Who:Are dedicated to enriching and improving the lives of others.Have a valid driver's license.Have basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.Possess a high school diploma or GED; for international students secondary education preferred but not required.Prior experience in Human Services field is preferred but not required.Must have current First Aid Certification.Must have CPR certification within one month of hire.Must complete MAP certification within 90 days of hire.Required EducationHigh School diploma and GED; for international students secondary education preferred but not required.Required Qualifications
