Illumination Foundation(IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The Associate Director of Healthcare Services will provide leadership and be responsible for the oversight of the recuperative care program, referral team and all healthcare programs at Illumination Foundation including CalAIM programs. This position will collaborate with regional operations leadership at IF to achieve organizational goals and improve the care delivery process. Home base is at the recuperative care sites. The Associate Director of Healthcare Services works across our Recuperative Care Programs, to provide post-hospital care, while creating a healthy, safe, and home like environment for client quality of life.The Associate Director of Healthcare Services has excellent bedside manners, is a team player and has a “whatever it takes” attitude. The pay range for this position is $70,000 - $85,000 annually, depending on experience. Responsibilities

Serve as a key member of the leadership team by developing and implementing new policies and programs, guiding plans for programmatic expansion through community and hospital outreach

Collaboratively lead initiatives to improve client safety, quality, infection control, risk management and client satisfaction.

Create and implement standard operating procedures and policies within IF’s medical recuperative care program.

Facilitating hospital discharge process and patient assessment/engagement program.

Monitoring medical assessment intake and discharge plan of care.

Oversight of current client chart via existing CRM database

Implement medical/wellness checks, medication regimen education, preventative measures and medical insurance follow up.

Participate in internal/external case conferencing, board review team meetings, staff meetings, trainings, and in-services.

Track and audit client information in existing database specific to program and plan of care.

Supervise and develop the Care Coordination and Medical Team by fostering accountability, promoting collaboration, strength-based leadership, and building a clinical competency training and development program for staff.

Collaborate with the administrative team to develop systems that facilitate an efficient and comprehensive care environment for clients and staff.

Travel to/float to Recuperative Care Sites as needed

Serve as a primary clinical contact with community partners by building partnerships with nursing schools and other healthcare-related academic institutions to advance IF’s mission and improve client care and care coordination.

Complete additional tasks/duties as required to meet the needs of the recuperative care programs or IF, not to impede primary responsibilities.

Preferred Experience/Minimum Qualifications

Required:

Valid California license as a RN

2- 5 years (minimum) relevant management experience in Healthcare Administration.

Knowledge of the nature and causes of homelessness

Current valid CA driver's license, Have a dependable vehicle with automobile insurance.

Computer literate; proficiency with MS Access, Word, and Excel.

Proven ability to stay calm and focused when working in a fast-paced environment.

Experience working with substance abuse and dually diagnosed clients, as well as patients with diverse socio-economic, ethnic, and cultural backgrounds.

Must be organized, detail oriented, can handle confidential information, and able to interface effectively with all levels of management.

Preferred:

Recent work experience with homeless patients.

Experience/background with CalAIM and/or care management.

Bilingual in English & Spanish
