Responsible for assisting individuals with intellectual/developmental disabilities to increase positive social functioning and in maintaining self-determined lifestyles. Collect information from clients and targeted contacts and maintain service documents. Coordinate effective communication between all invested persons to enhance client’s service delivery. Monitor the implementation of services and ensure appropriate resources are provided.

Essential Functions

Coordinate information from clients and support team and consolidate into the person-centered support plan and other service documents.

Assist client in selecting and obtaining services that they have chosen to enhance functioning; support planning.

Organize and monitor delivery of selected services, intervenes when client self-determined services may be deficient and provides referral and transitioning tasks as needed.

Directs diplomatic communication with client, family members, service staff and other helping professionals to ensure appropriate services and most complete review of services provided.

Address needs, issues and concerns of client as directed by K.A.R. 30- 63-24.

Document services, activities, actions and plans for the client.

Advocate for client’s preferred lifestyle and services.

Job Qualifications and Requirements

Education in Human Services, Psychology, Social Work or other area for helping professions. Experience working with people with intellectual/developmental disabilities. Ability to work with individuals from diverse backgrounds, cultures and perspectives. Communication skills that enhance cooperation. Team oriented and resilient attitude.
