Hours: Full Time

Pay Range: $45,000-$55,000 annual salary

Company Description: Crossroads Aging Care Professionals, LLC provides care management, advocacy, consultation services, and serves as surrogate decision makers for older adults, people with disabilities, and people with behavioral health challenges.

Job Description and Duties

Seeking a full time Care Manager to work with older adults, people with disabilities, and people with behavioral health challenges. Requires working in the field, primarily in client homes or in an office setting. The Care Manager is responsible for conducting psychosocial assessments to identify client needs, develop client-centered plans of care, and assist with care coordination tasks. The Care Manager should also be able to develop rapport and build relationships with clients and families, while maintaining an awareness of appropriate, professional boundaries and ethics. Respectful and effective verbal and written communication with clients, families, medical and legal professionals, as well as other service providers are essential. Documentation of all client interactions and report writing is required. When Crossroads is the designated decision maker, the Care Manager gathers needed information and evaluates treatment options, communicates with medical professionals, and makes decisions in line with client wishes and preferences. Coverage of evening and weekend client needs through a rotating, on-call schedule.

Qualifications

A Baccalaureate or master’s degree held in a field related to Care Management, including nursing, social work, gerontology, behavioral health, occupational therapy, or physical therapy.

At least 2 years of experience primarily engaged in the direct practice, administration, or supervision of client-centered services to older adults, people with disabilities, and/or people with behavioral health challenges. Experience working in the community is preferred. Experience working with clients and families through crisis is preferred.

Current Care Manager Certified (CMC) and or Nationally Certified Guardian (NCG) certificate holder or meets the requirements to become certified.

Potential candidates must be self-motivated, resourceful, able to work autonomously while managing a caseload of individuals and families with a variety of co-occurring needs.

Must be knowledgeable about health and disability, community resources, insurance and entitlement programs, housing, family dynamics, and advocacy.

Comfort with technology and basic computing.

Must have a functioning vehicle.
