The Behavioral Health Care Manager is responsible for supporting and coordinating the behavioral healthcare of patients on an assigned caseload with the patient’s primary care team, which includes providing psychotherapy and behavioral interventions within the primary care setting. BHIP (Behavioral Health Integration Program) is a professional team using the UW AIMS Collaborative Care Model to improve access to evidence-based behavioral health treatment at Island Hospital. We focus on supporting mental health care within primary care for patients with a range of common mental health symptoms and diagnoses within a fast-paced primary care environment. The BHIP Care Manager is an integral member of this team, working alongside the patient’s medical provider, psychiatric consultant, as well as the larger primary care and medical teams.

Requirements:

- Master’s Degree in Social Work

- Active Social Worker Independent Clinical License (LICSW) issued by the WA Department of Health

- Current BLS certification

- Previous experience in an outpatient setting desired

Skills/Qualifications:

- Excellent verbal and interpersonal skills to interact positively with a wide range of people.

- Experience with screening for common mental health and/or substance abuse disorders as well as experience with assessment and treatment planning for the disorders.

- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.

- Ability to effectively engage patients in a therapeutic relationship, when appropriate.

- Ability to work with patients by telephone or telemedicine as well as in person.

- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).

- Ability to multi-task and prioritize tasks, and quickly synthesize information.

- Ability to work in diverse, multicultural settings with a demonstrated commitment to non-discriminatory and equitable care.

- Ability to work effectively both as a large, interdisciplinary team while also working independently with minimal supervision.

- Proficiency in the use of MS Word, MS Excel, MS PowerPoint, and MS Outlook.
