Social Worker-QDDP

The Qualified Developmental Disabilities Professional (QDDP) is responsible for providing oversight of the individual’s services and person-centered plan. QDDP supervises the provision of personal care, companion, respite, and crisis intervention services to the person, evaluates the continued appropriateness of such services, and makes changes when the individual’s needs or desires are not being met. The QDDP conducts a site visit at least every ninety (90) days, and more often if needed. The QDDP makes assessments of the effectiveness of the service, person/family satisfaction with the service, and institutes any changes that may need to be made.
