As a Medical Assistant you will provide oversight of care coordination for physician practices. The coordinator will facilitate and document the transfer of information between providers and patients, as well as access to services needed. As the doctor and patient liaison you will communicate effectively establishing a relationship of trust and support that continues over time as needs of patients change.

Job Responsibilities:

Responsibilities include documentation of each client interaction, assisting in making sure provider has proper updated patient records as required per program. Accountable to deliver exceptional customer service in accordance with our mission, vision and values. Model the our culture of excellent customer service with patients, caregivers, families, visitors, facility staff, and other team members.

Other Responsibilities:

Assist in the communication process of following primary medical care plan and addressing chronic health conditions.

Develop rapport with patients through respectful outreach and client-centered engagement practices.

In collaboration with the patient's primary provider care plan, implement individualized patient standardized strategic goals. Review and update this plan regularly to reflect progress and attainment of articulated goals. Document emergence of new patient needs and achievement of new goals.

Assess nature and severity of patient health. Document and coordinate between primary individual's health changes.

Assist patients in keeping appointments.

Assist with accessing needed services by setting up and attending appointments with patient in order to provide continuity of care.

Identify gaps and barriers in available community resources and advocate for systemic changes.

Develop and maintain client files for assigned caseload according to company protocol, contract and state requirements.

Additional duties as assigned by care management team and providers may be required.

Education

Certified Medical Assistant Only.

Experience

A minimum of two years of experience as a Medical Assist or medical office work is preferred.

Knowledge Requirements

Computer literate, with basic knowledge of Microsoft Office Suite.

Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.

Dependable, able to work under pressure, receptive to change, willingness to learn, cooperative approach to problem solving.

Ability to establish and maintain effective working relationships with staff, patients, and outside contacts from a wide variety of ethnic, cultural backgrounds.

Excellent attention to detail.

Excellent Customer Service skills and telephone skills.

Ability to work very independently and seek supervision when appropriate.

Knowledge and understanding of Medical Terminology.

Language

Bilingual a plus.
