JOB TITLE: Unit Clerk

DEPARTMENT: Acute and Critical Care

REPORTS TO: Acute and Critical Care Director

JOB SUMMARY: Responsibilities include: greeting patients and visitors at the desk in a friendly, courteous manner; coordinating nursing staff's duties; assisting with admissions, transfers and discharges; entering orders into the Kardex; entering charges into the computer; ensuring charts are complete and up-to-date; ordering supplies for nursing area, dietary area and desk; answering phones and patient call lights; paging throughout hospital, and other administrative duties.

JOB REQUIREMENTS: High school diploma preferred. CNA and Unit Clerk Certification preferred.

EXPERIENCE: Experience in secretarial training, switchboard, medical terminology, nurse's aide work, office machine operation preferred.

SKILLS: Excellent communication skills. Must be able to maintain a high degree of accuracy under stress, complete monotonous detail work without error, and handle several tasks at once. Must be competent with Microsoft Explorer, Word and Excel, and willing to learn medical programs as required.

ESSENTIAL FUNCTIONS

Unit Duties:

  • Greets patients, visitors and staff in a courteous and professional manner. Presents a positive image, while directing patients, visitors and staff to appropriate areas. Helps to enforce applicable visiting policies under direction RN/LPN.
  • Manages scheduled and emergency unit lockdown.
  • Answers patient intercom promptly and professionally; refers urgent requests to RN/LPN immediately.

  • Answers telephone efficiently and professionally; directs calls or takes notes as appropriately.

  • Operates communication equipment at desk. Activates Code Blue, Pink and White protocols. Activates surgery and trauma cases as required. Manages staff emergency response line. Notifies individuals as directed over hospital intercom, pager system, or phone. Communicates with other departments, as needed.

  • Acts as hospital-wide switchboard after 6 PM.
  • Performs registration duties after hours and when PFS is not available.

  • Night unit clerks enters charges from the daily charge sheets into the computer. Daily charges are checked against the nursing schedule.
  • Prints staffing for the incoming shift, and places a copy in each nurses station
  • Orders equipment and supplies. Keeps desk stocked with needed clerical supplies. Orders supplies for supply rooms. Keeps inventory to desired levels.

  • Enters work orders in computer as needed.
  • Monitors alarm systems at the nurses’ station.
  • Monitors of Meaningful Use, Core Measure, HCAHPS and safety criteria and implementations as assigned by the director.
  • Manages and tracks discharge phone calls and chart abstracts
  • Enter vital signs, intake and output, and diet intakes for CNAs or nurses, time permitting
  • Promotes positive attitudes between nursing and other departments. Maintains a positive attitude toward co-workers, patients and visitors.
  • Notifies appropriate departments of maintenance alarms.

  • Admissions, Transfers, Discharges, Orders, Medical Records and Patient Care
  • Coordinates admission of patients. Completes proper admission documents. Coordinates room assignments under the direction of the House Supervisor. Adds patient to computer records and initiates labeled medical records on admission.

  • Enters orders into the Kardex. Refers telephone, verbal and written orders to RN/LPN. Updates diets for dietary services. Requests diet changes, snacks, and care partner or guest trays from dietary. Informs CCU of telemetry orders and telemetry discontinued orders.
  • Coordinates discharge/transfer of patients. Notifies pertinent departments of patient's discharge, makes follow up appointments as ordered, completes referrals, and prepares medical record for discharge. Prior to patient discharge, notifies nurse of receipts for valuables, clothing or medications which have been placed in lockup. Notifies EVS to clean room after discharge.

  • Maintains medical records in an orderly manner. Obtains past medical records under direction of RN/LPN or Provider. Prepares medical records for transfer when necessary. Transfers patient information to directed recipient.
  • Coordinates activities of nursing with other departments. Notifies other departments of pertinent changes in status or physician orders.
  • Assists with recording of patient information in medical record. Notifies RN/LPN if information is missing.

SECONDARY FUNCTIONS

  • Maintains competency skills as directed by the Nurse Manager.

  • Participates in monthly staff meetings and committee assignments intended to improve patient care.
  • Regularly attends departmental and hospital meetings and educational in-services.

  • Performs other duties as assigned.
