RN - Patient Services Coordinator

Providence is calling a Registered Nurse (RN) Patient Services Coordinator (Forest View, Variable, 0.0 FTE) to Providence Forest View in Anchorage, Alaska. In this position you will:

Performs care coordination/case management tasks for patients as needed (coordinating with MD's, families, home health etc.) Assists and discusses with patients any areas of concern and assists patients with medications and treatments as stated in the service plan. Reviews and performs vital sign checks. Documents essential information in patient’s chart.

Works with a team approach to complete daily operational needs and provides leadership to ensure work processes are within standards.

Promotes and routinely offers comfort and pain management measures. Documentation reflects assessment, treatment and re-assessment as required to meet patient goals.

Safeguards all forms (Electronic, written and oral) of confidential information as it relates to patients, their families, medical staff and employees. Is aware of and compliant with organizational polices regarding Fraud and Abuse, Conflict of Interest and the Code of Conduct.

Coordinates staffing to meet patient needs on a day-to-day basis. Ensures PG’s and other staff understand their assignments and are able to perform them to established standards.

Participates in staff meetings and promotes the department quality improvement process.

Responds to, and reports, real or simulated emergency and disaster situations according to system procedure.

Completes floor restocking order sheets.

Qualifications:

Required qualifications for this position include:

Graduate of an accredited approved School of Nursing

Licensed as a Registered Nurse (RN) in the State of Alaska

Current Basic Life Support (BLS) certification upon hire

Strong assessment skills

Preferred qualification for this position include:

Prior experience in orthopedics

Forest View is a short stay facility and is licensed by the State of Alaska as an Assisted Living Facility, located in Building S of the Providence Health Park on Piper Street. Forest View provides a rapid recovery environment for patients requiring a variety of nursing and physical therapy services for a short period of time following their surgery at one of the local surgery centers in Anchorage.
