I. Job Summary:

The Reentry Case Manager will provide support for individuals reentering the community from incarceration. They will act as the case manager for reentrants by coordinating with the Department of Corrections (DOC) on developing the Offender Management Plan for eligible reentrants. This person will assist in connecting reentrants to services such as Medicaid (or other health insurance), medical, dental, housing, transportation, employment, mental/behavioral health, substance use treatment, education, and food security. Contact with reentrants may begin up to 90 days prior to release and continue for one year post-release.

The Reentry Case Manager will work with community partners to identify and address gaps in services and barriers facing those that are releasing from incarceration. This will include regular visits and communication with community stakeholders to ensure up-to-date information on the services they provide. Advocacy and outreach will be performed regularly to help build relationships and bridge the gaps to service.

IV. Primary Duties & Responsibilities:

● Provide in-reach services within jails and prisons to inform inmates about the Reentry Program.

● Meet with the Institutional Parole Officer and inmate 90 days prior to release to assess the inmate’s needs and develop the Offender Management Plan.

● Provide case management services identified by the Offender Management Plan to connect the reentrant to the resources necessary to successfully transition from incarceration back to the community.

● Meet with the Field Probation Officer 30 days prior to release to develop a plan of action for the inmate upon release.

● Work with the Field Probation Officer to coordinate care for the reentrant once they are released and meet with the Field Probation Office for regular staffing meetings.

● Coordinate wrap-around services by connecting the reentrant to community partners, service providers, and other service organizations through referrals. Schedule regular staffing meetings with each service provider on a schedule to be determined by the Case Manager and the service providers.

● Maintain detailed records for each reentrant served by the Reentry Case Manager and generate reports as needed or requested by the Board of Directors, State of Alaska, and the DOC.

● Access and maintain documentation within the Alaska Automated Information Management System (AKAIMS).

● Access and document referrals through Alaska Corrections Offender Management System (ACOMS).

● Collaborate with community agencies, services providers, local and state government, and other service organizations to identify and address gaps in services and barriers facing reentrants.

● Form an intake packet utilizing agency information, required forms for DOC and state agencies, and other relevant forms for providing services.

● Provide advocacy for Restorative Justice programs and initiatives and advocate on the behalf of individuals that are transitioning from incarceration back to the community.

● May assist in the planning and implementation of Coalition events and activities.

● Meet with the Board of Directors monthly to provide program updates.

● Attend monthly Board and organization meetings.

● Attends bi-weekly meetings of Reentry Case Managers coordinated by the State of Alaska. This meeting may be attended telephonically or by other telecommunication means.

● Provide input on the organization's social media account and website in partnership with the Board of Directors.

● Maintain a dedicated Reentry Case Manager electronic mailbox.

V. Qualifications:

● Bachelor’s degree or greater in Social Work or a closely related field, or an equivalent combination of education, job specific experience, certifications, and licenses that provide the knowledge, skills, experience and competencies required to successfully perform the essential function or the job.

● Valid Driver’s License and regular access to reliable transportation.

● Must pass a background check for entry into DOC facilities and gain access to records and referrals.

● Proficiency with computers and experience with data entry.

● Proficiency in Microsoft Office Suite programs, Google Drive, Google Docs, and Adobe programs.

VI. Competencies:

● Communication: The Reentry Case Manager must have excellent communication skills (both written and verbal) to facilitate collaboration among all stakeholders.

● Commitment: The Reentry Case Manager must show commitment to Restorative Justice practices and theories, as well as a commitment to the success of the reentrants.

● Accountability: The Reentry Case Manager must be accountable to multiple stakeholders including, but not limited to, the Board of Directors, Fiscal Agent, Department of Corrections, Department of Health & Social Services, Coalition members, community partners, and the reentrants.

● Responsibility: The Reentry Case Manager must be self-directed and be proficient at working with minimal supervision. They must keep their commitments, especially the ones made to the reentrant. This employee is expected to maintain regular contact with their direct supervisor and with the Board of Directors. Once trained, this person will be expected to understand the duties of the position without daily oversight.

● Ethics: The Reentry Case Manager will practice the principles, values, and ethical behaviors associated with those working in the helping professions and with vulnerable populations. These values and ethics include, but are not limited to, respecting the diversity and life experiences of the reentrant, maintaining confidentiality and client privacy, maintaining professional boundaries with clients, and avoiding any exploitative practices in their relationship with clients.

VII. Typical Physical Demands:

● Occasional bending, stooping, and stretching.

● Requires good hand-eye coordination and manual dexterity sufficient to operate both Mac & PC computers, phone and/or tablet, and other portable devices.

● Requires normal range of hearing and eyesight to record, prepare, and communicate in writing, in person, and by phone.

● May require limited lifting up to 50lbs.

● The position requires that the individual be comfortable working in a prison or jail environment and in close proximity to inmates.
