POSITION: Program Manager (Habilitation – English & Spanish Caseload)

DESCRIPTION: Manage in-home services for approximately 40-50 clients. The majority of clients on the caseload are receiving Habilitation services. Habilitation services involve teaching our clients a variety of daily living, self-help, and safety skills so that they may lead more independent, self-directed lives Therefore, the ideal candidate for this position will have experience in developing and implementing teaching strategies. The clients on the caseload may also be receiving a variety of other services such as personal care, respite, and housekeeping. Duties of the manager include:

1. Working Mon-Fri 8a-5p. Please note the position will occasionally require the manager to work a different schedule to accommodate client needs. This may include working direct care shifts, some evenings/ weekends.

2. Performing on-call duties at least once every 10 weeks. The duties last for a duration of 2 consecutive weeks. These duties include answering and managing emergency phone calls from 5p-8a M-F and all day Sat and Sun. These duties may also include working direct care shifts.

3. Developing and guiding direct care staff in implementing strategies for teaching clients self-help and independent living skills.

4. Maintaining and growing a caseload.

5. Scheduling shifts for clients and direct care staff.

6. Orienting and training direct care staff with clients.

7. Monitoring direct care staff performance.

8. Holding staff meetings as needed and attending weekly program department meeting.

9. Conducting initial intake of new clients.

10. Meeting with clients/families minimally on a quarterly basis.

11. Attending meetings with other agencies involved in a client's care

12. Ensuring that all paperwork in client files is updated regularly.

13. Documenting all significant conversations and/or correspondences with staff and clients.

14. Maintaining client and agency confidentiality at all times.

QUALIFICATIONS:

1. 18 years of age or older

2. Bilingual in English and Spanish

3. 1+ year teaching/Habilitation experience

4. Ability to obtain a current CPR certification

5. Ability to obtain a current First Aid certification

6. Ability to obtain fingerprint clearance

7. Ability to clear a DES background check

8. Ability to obtain a current T.B. clearance card

9. The ability to work in a team environment

10. The ability to handle difficult situations with clients/families

11. The ability to work a flexible schedule

12. The ability to learn and operate UCPSA program software

13. The ability to work independently

14. Valid driver's license and reliable transportation

15. The ability to lift and transfer clients
