As a Supported Living Specialist at Community Connections, you can make
a real impact on the daily lives of adults with developmental disabilities.
Responsibilities:
The Supported Living Specialist is the primary advocate for all consumers on their caseload, encouraging independence and personal growth, while ensuring optimal well-being, and providing on-going support, supervision and training in all areas of daily living. Working closely with Program Coordinators, Managers, and other Supported Living Staff in the development and implementation of all services provided to individuals. Documenting daily field notes in Icentrix. Position requires On-Call rotation of 24 hour emergency service responsibilities.
Qualifications:
High school diploma or general education degree required
Clear verbal and written communication skills
Previous human services, behavioral or medical experience preferred
Extensive training/working/volunteering/or has a family member with disabilities
Valid driver’s license and 3 years of satisfactory driving experience
Independent means of transportation
Attentive to the changing needs of consumers
Working knowledge of Smart Phone and Chromebook technology preferred
Ability to work flexible hours including evenings and weekends
Bachelor’s degree in related human service or business field preferred
Certificates, Licenses, and Registrations Required: Must be certified in CPR and First Aid and complete Human Rights and DPPC training (training can be provided). Must possess a valid driver’s license and have a registered automobile and current automobile insurance.
