Basic Functions :

Provide direct nursing care to the residents. Supervise the day-to-day nursing activities performed by the NAC's. Such Supervision must be in accordance with all current federal, state, and tribal standards, guidelines, and regulations governing the facility.

Provide the highest degree of care to each and every resident. Qualifications

MINIMUM QUALIFICATIONS :

Education & Training :

Must possess a current license as a Licensed Practical Nurse (LPN) in the state of Washington AND have thirty-six (36) months of work related experience.

Must be knowledgeable of nursing, medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

Must report abuse neglect to state hotline as a mandated reporter.

Must possess the ability to make independent decisions.

Willing to cover extra shifts when able, as this facility operates 24 hours a day, seven days a week.

Requires a 1st aid / CPR card or able to obtain within first 90 days of employment.

Requires a TB test with negative results or obtain within 30 days of employment.

Must successfully clear an extensive Criminal History Background Inquiry and maintain throughout employment.

Must maintain a State Drivers' License and be eligible for the Tribes' Insurance throughout employment.

Knowledge, Skills, and Abilities :

Participates in admissions, transfers, and discharges of residents.

Human Resources duties include supervision, training, assists DNS with evaluating, training, disciplining, and scheduling.

Reviews, transcribes, and implements physician's orders.

Participates in resident assessment and care plan activities.

Communicates with physicians, making rounds as needed.

Alerts physicians of condition changes in residents.

Makes nursing rounds to assess residents and evaluate care.

Checks for appropriate amount and condition of equipment and supplies.

Checks for appropriate amount of condition of equipment and supplies.

Ensure that all nursing service personnel comply with the procedures set forth in the procedure manual.

Completes medical forms, reports, evaluations, studies, charting.

Completes narcotic counts for the shift.

Does timely assessment on admission and as needed (i.e. weights, nutritional, fluids, pain, skin and behaviors).

Chart nurses notes clearly and concisely reflecting the care provided, as well as the resident's response to the care.

Able to perform lab draws, wounded care, infection control procedures, IV fluids, Foley catheterizations, peg tubes, and PIC site monitoring

Record new / changed diet orders. Forward information to the dietary department.

Fill out and complete transfer forms in accordance with established procedures.

Phone / fax lab results to physician and any consulting physician's orders.

Coordinate the information associated with a new resident admission. Participate in the orientation of the new resident / family to the facility.

Notify the physician and responsible party(s) when there is a change in the resident's condition and / or death. Call funeral homes when requested by family.

Ensure that established post-mortem procedures are followed.

Monitor / assess critically ill residents as necessary.

Ensure that residents who are unable to call for help are check frequently.

Deal with staffing issues re : Nurses, NAC / NAR and other departments on weekends.

Emergency preparedness requirements in the Long Term Care setting.

Must be able to read, write, and understand the English language.

Must be knowledgeable in Geriatrics and LTC setting on Chronic Diseases.

Must be able to work and think independently.

Must respect resident rights.

Must be able to push, pull, move, and / or lift a minimum of fifty (50) pounds to a minimum height of three (3) feet and be able to push, pull, move and / or carry such weight a minimum distance of fifty (50) feet.

Must be able to work all shifts, weekends, and holidays.
