SKIP of New York is a leading not-for-profit organization supporting medically fragile children and youth with a variety of physical and developmental disabilities. Launched over 35 years ago, SKIP believes every child deserves the best childhood they can get. SKIP connects children/youth with supports they need (like nursing/home care, insurance coverage, DME, supplies, respite, environmental modifications, grants, etc.) to heal and thrive, living at home with their family rather than in hospitals and institutions. SKIP' services allow children to participate in their schools and communities and to realize their full potential. Through care management, intensive advocacy and hands-on problem solving, SKIP Care Managers make sure the children they work with each year get that chance.

SKIP wants to grow to help even more families across NYS, so we are seeking exceptional and caring Care Managers for our offices in Painted Post/Southern Tier, Rochester and Buffalo. Care managers provide advocacy for the child and family, ensure access to all needed services, make referrals to community-based services such as doctors and support groups, and monitor all services for satisfaction. The care manager is also responsible for developing and maintaining a plan of care for each individual, conducting face-to-face site visits with individuals monthly in various locations across the State, and maintaining required paperwork to ensure continued regulatory compliance. A reliable vehicle with appropriate insurance coverage is required. Ideal Candidates must have at least a Bachelor's Degree, preferably with 2+ years of care management/early intervention/human services related experience. Compensation commensurate with education/experience.

Qualifications;

Minimum Education required: Bachelors in the health or human services field

Two years of case management experience within the developmental disabled population.

Bilingual a Plus
