POSITION: Job Coach for Transitional Employment

STATUS: Full-Time; 37.5 hours / week OR Part Time; 20 hours / week available

LOCATIONS: Monroeville

SCHEDULE: Monday through Friday, 12:00 PM to 7:30 PM (Full Time) OR 3:30 PM to 8:30 PM (Part Time)

WAGES: $13 / hour

Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

The Job Coach for Transitional Employment, internally referred to as the Crew Leader, will be responsible for providing support and supervision to participants transitioning to competitive employment through Small Group Employment in a 1:1, 1:2 or 1:3 ratio. Responsibilities include, but are not limited to:

Providing training, supervision and support to Small Group Employment Participants to promote skill growth and independence

Providing transportation, via Agency vehicle, for participants to sites when needed

Timely completion of all Participants’ paperwork, including, but not limited to, timesheets (time clock system), Daily Service Notes and Monthly Progress Notes

Ensuring that service provided is specific to the participant’s ISP and meets their needs and preferences

Participating in team meetings and the development of the ISP

Monitoring each participant’s progress towards specific outcomes, identifies lack of progress and develops corrective action plans to help the participant meet their goals

QUALIFICATIONS

1. High School Diploma

2. Two (2) to Three (3) years’ experience in human services, preferably in Vocational training with people with disabilities

3. Valid PA Driver’s license

4. Reliable transportation

5. Working knowledge of MS Office software

6. Certified Employment Support Professional (CESP) credential (ACRE training and certification must be completed within 6 months of start date

7. Act 33, 34 and (if applicable) FBI clearances

WAGES

$13 / hour
