"IMMEDIATELY" LPN MEDICAID PRIVATE PROVIDER

24.72 Per hour

CRITICAL NEED MUST BE RELIABLE, PROFESSIONAL, RESPONSABLE , PUNCTUAL, MATURE, CARING COVER OTHER SHIFT IF IS NECESSARY

Non-smoker. Patient is highly allergic

4 DAYS A WEEK 12:00PM TO 11:00PM

Must be familiar with Mickey G feeding tubes;

doing all aspects required of a nurse.

Must have two references

non-expired State Identification.

Must already have Medicaid private provider number.

Required license or certification:

Driver's License

Social Security Card

private provider for Medicaid with an active NPI#.
