Both CHNw and DHS place high importance on continuing and advancing population health and value-based care to improve patient health outcomes.Make a DifferenceThe Innovative Healthcare Collaborative of Indiana (IHCI) is seeking a Clinical Program Development and Education Specialist to join our population health team. Our goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system. As a member of the team, you will have the opportunity to make a profound impact on the lives of people living with complex and/ or chronic conditions. This position may work remotely or in-office. The worksite will be up to the candidate. There will be occasional mandatory in- person meetings which may require travel up to three hours. The Clinical Program Development and Education Specialist will be responsible for providing clinical education and training to IHCI Clinical Staff. This individual will collaborate with Clinical Leadership to identify training needs, complete chart audits, design and implement educational programs, and evaluate program effectiveness. At all times, the Clinical Program Development and Education Specialist will uphold the IHCI values of Respect, Teamwork, Integrity, Continuous Learning, Innovation, Patient Centered, and Providing Value.

Development and maintenance of IHCI proprietary clinical programs for Commercial and Medicare Populations.

Stay up to date on evidence-based practice guidelines for Disease specific care management programs for Commercial and Medicare populations.

NCQA certification research, programming development and implementation, and monitoring and maintenance of NCQA requirements.

Routine Chart audits and reporting tool oversight for care programs and care advisor performance monitoring.

Collaborate with Analytics and Clinical Leadership to develop and implement automation process for auditing.

Evaluate the effectiveness of educational programs and make necessary modifications to improve outcomes.

Develop ongoing skills training to support professional development.

Create and maintain training materials, including patient disease specific education packets, program process guidebooks, Tip sheets, and check lists.

Develop, implement and in collaboration with clinical leadership, monitor new hire orientation training including materials.

Develop, implement and in collaboration with clinical leadership, monitor preceptor program.

Provide clinical mentoring and support, as needed. Ensure compliance with all relevant NCQA regulations and guidelines.

Project management for any new programming.

Collaborate with vendors such as Health Catalyst, to create IHCI proprietary care programs in the vendor IT platform.

Collaborate with Clinical leadership to develop necessary training materials for new care programs.

Coordinate and deliver new employee orientation, onboarding and training, including post training follow up.

Measure, improve and maintain quality outcomes (clinical, financial, and functional) for care programs and the population served.

Individuals may not perform all of these duties, or may perform additional, related duties not listed here.

Exceptional Skills and Qualifications

4 year / Bachelor's Degree in Nursing; or a Master's degree in Social Work (MSW) ( Required )

Master’s degree in nursing (Preferred)

One of the following is required Certifications/Licensures: (Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) , or Licensed as a Social Worker (LSW) by the Indiana Professional Licensing Agency (IPLA) ( Required )

1+ years: CCMC (Commission for Case Manager Certification) or other relevant Case Management Certification (Preferred)

1+ years: Knowledge and experience teaching population health patient self-management. (Preferred)

1+ years: Knowledge and experience with program development, IT integration, and multisite education and new programming launch. (Preferred)

1+ years: Knowledge and experience with NCQA certification process and requirements (Preferred)

1+ years: Experience working with a Population Health Team &/or Accountable Care Organization ( Required )

1+ years: Experience in Informatics and/or as an EMR analyst (Preferred)

1+ years: Project management experience ( Required )

3+ years: Demonstrated expertise with EMR, Microsoft Office and other IT tools ( Required )

3+ years: 3-5 years of nursing experience, preferably in-home health, ambulatory care, community public health, case management, coordinating care across multiple settings and with multiple providers ( Required )

3+ years: Medical or community Social Work experience providing patient-centered outreach, behavioral health services, needs assessment and support. ( Required )

3+ years: Experience providing social work services within a variety of population health and/or value-based care program setting. (Preferred)

5+ years: Experience in a clinical setting, with experience in nursing education. (Preferred)

Demonstrated experience in designing and delivering training programs. ( Required )
