Assists YVFWC clients in obtaining the social and health services they need in an integrated and coordinated manner.What we are looking for:A compassionate, caring, person who understands social determinants of health and has the desire to work with maternal clients throughout the pregnancy cycle and with pediatric clients.

A Case Manager who is passionate about community health.

A clear communicator who is able to build strong relationships with clients, clinic staff, and work within a team.

What you will do:Work in collaboration within a First Steps care team including nurses, social workers, case managers, and dietitians.

Provide resources and referrals to aide YVFWC clients in obtaining needed social and health services in an integrated and coordinated manner.

Document the care provided and collaborate with the care team during case reviews.

Build relationships, trust with clients, and see the results of the direct positive impact you have made in their lives.

Qualifications:Education: Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology or Child Development.

Experience: Two years of experience working in community social services, public health services, crisis intervention, outreach or linkage programs.

Licenses/Certificates/Registration: Current Washington driver's license and proof of automobile liability insurance coverage.

Knowledge/Skills/Abilities Required: Basic bilingual proficiency in verbal and written communication preferred. Must have demonstrated ability to communicate at level 10 on the ALTA language proficiency scale in order to receive bilingual differential. Effective verbal, written and listening communication skills. Strong customer relations skills including the ability to manage stressful situations. Knowledge of medical terminology required. Knowledge of cultural differences and impact on appropriate care provided to patients. Knowledge of local social and health services. Knowledge of how to access local community agencies and resources. Ability to develop effective working relationships with social service providers and partners. Ability to effectively interact with a variety of clients and employees in a professional manner. Basic proficiency with a variety of computer programs including, Microsoft Outlook, Word and Excel.
