Home Health NAC

Role Summary:The NAC will provide individualized patient care integrating age related, cultural, and religious/spiritual needs. The care delivered reflects the philosophy, goals and objectives of the hospital and nursing services. The NAC is accountable for own practice.NAC-Home Health/HospiceAble to work independently in home settings providing personal care, light housekeeping, light meal preparation. Able to do five to six home visits in an 8 hour shift. Understands how to adapt care in home setting, recognizing when practices may not be safe for the patient or the employee. Alerts the patient or family when going to be later than expected for the visits. Works under the oversight or supervision of a RN, PT or OT who will establish a plan of care for each individual patient.Experience: Minimum of one (1) year healthcare experience required.Certifications/Licenses: Ability to pass Home Care Certification test within 90days of hire. Current WA state driver's license and proof of insurance.Education: Complete twelve (12) hours of Home Care education credits (provided by employer).Additional requirements: Cell phone for use during working hours, car for travel to and from home visits during shift.At Virginia Mason Memorial, our vision is to "create healthy communities one person at a time."
