PRIMARY RESPONSIBILITY

The Resident Care Coordinator (RCC), in collaboration with the Executive Director, coordinates and supervises direct care staff to deliver care which will ensure that resident’s needs are met through resident service plans, physician orders, community policies and procedures, and federal, state and local regulations. The Resident Care Coordinator reports directly to the Executive Director and collaborates with the Wellness Director.

ESSENTIAL JOB FUNCTIONS

Provides oversight of care implementation by direct care staff; recognition of deviation from the service plan.

Medication Management, including oversight of medication storage and administration. Carry out physicians’ orders in a timely manner.

Participates actively in the Primrose community’s quality improvement process, including regular attendance at, and reporting to the Quality Committee. Participates in ongoing facility efforts to improve the overall quality of the nursing care, including efforts to evaluate and address the causes of various care-related problems and deficiencies.

Knowledgeable of state and other applicable regulations and demonstrates ability to implement and maintain compliance with these regulations governing the community, resident care, and any applicable reimbursement.

Responsible for the appropriate delegation of non-licensed nursing staff as outlined by the State Board of Nursing and specific State regulations.

Proactively develops positive employee relations. Promotes teamwork, mutual respect, and effective communication.

Monitors care for residents receiving services through third party providers, assuring documentation requirements are met.

Maintains confidentiality within the workplace with all written and verbal communications pertaining to residents, staff, and community operations.

Assists with orientation of new staff to job responsibilities as needed.

Implements and contributes to the evaluation of the resident’s response service plan of care, medication and treatment plans.

Observes changes in resident status, needs or preferences and documents and reports appropriately. Contributes observations and health information to the Wellness Director.

If providing care in a Primrose Memory Care community, actively engages and assists in ensuring person-centered approaches with programming and behavioral expressions are carried out per Primrose Memory Care Program.

KNOWLEDGE/ABILITIES/SKILLS

Knowledge of current state laws and regulations that apply to the assisted living setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Demonstrates leadership skills in principles of team building, motivating others, and conflict management/resolution. Knowledge of Alzheimer’s Disease, other dementias and related memory impairments is preferred. Basic knowledge of computer software and programs required, minimum working knowledge of Microsoft Outlook and Word. Able to work primarily day hours with occasional varying hours and call time.

EDUCATION/EXPERIENCE

Current, active practical nurse license or other health related certification or license in the state the community is located in. Maintain CPR and First Aid Certification per state regulations. Meet annual continuing education hours required for nursing license if applicable. Experience in long term care or assisted living environment is preferred.
