The Field Care Manager Nurse for our Gateway center will work in clinic alongside providers and staff to assess discharge care coordination needs of patients, make telephonic follow up calls and coordinate services for identified members who have acute and long-term chronic conditions and or needs.NOT A REMOTE POSITIONJob Functions:Able to use nursing experience and judgment to anticipate members' care coordination needs

Refer complex patient concerns to the member's physician and medical director for appropriate interventions

Works closely with the hospital case managers, attendings, PCP and medical director, community agencies and other approved vendors to ensure the member's healthcare needs are met

Acts as resource and is able to facilitate eligibility and benefit information

Acts as a resource and is familiar with the provider network

Maintains accurate and concise documentation and adheres to documentation guidelines

Role Requirements:Experience with Medicare age population

Health Coaching & Case Management experience with Medicare age population

Active RN License

Active CPR-BLS from the American Heart Association

Minimum of 2 years of prior clinical experience in an acute or post-acute setting

Proficiency with computers including Microsoft products, Word, Excel, and Outlook

Able to provide face to face and telephonic patient care

Able to stand and walk for long periods of time

Preferred Education, Experience & SkillsPrior intensive care and/or coronary care or Medical Surgical experience preferred

Bachelor's or Associate's Degree in Nursing preferred

Experience with Electronic Medical Records; e-Clinical Works (e-CW)

ICD-10, CPT codes, HCPCS knowledge preferred

Managed care experience including knowledge of HEDIS and CMS guidelines

English and Spanish bilingual preferred
