The Charter House Coalition, Inc. seeks an energetic and detail oriented LICSW or Licensed Mental Health Clinician to join our team as a Clinical Case Coordinator. The ideal candidate will hold a license with the State of Vermont, will have at least 3-5 years of experience providing clinical assessment and case management and will have worked collaboratively with others in the delivery of human services. Those with experience working with the homeless population will receive preference but those with a compelling desire to bring our mission into action will also be considered.

The Clinical Case Coordinator will develop relationships with clients to connect each individual with ongoing services available through our partner agencies and will work closely with case management staff to establish and support housing sustainability plans. Providing an interim level of counseling services including supporting clients experiencing mental health crises will be a main component of this role. The Clinical Case Coordinator will also provide ongoing mental health education and training for staff and volunteers, participate in organizational goal setting and planning, and will share on-call duties as necessary.

The Charter House Coalition provides emergency shelter, healthy meals, basic essentials, housing services, case management, interim counseling, and on-going outreach support to recently housed clients. Our mission as a low barrier shelter is to meet clients where they are, regardless of any complex needs. We work with our clients to set goals to improve personal well-being and stability in order to successfully find permanent housing. The Clinical Case Coordinator will be lead member of the team responsible for providing these services.
