Founded in 1837, Cuyahoga County’s safety-net health system operates four hospitals, four emergency departments and more than 20 health centers.Summary:The Virtual Care Nurse provides individualized care using digital health tools to communicate to patients in real-time. The RN will support patients with both acute care needs and ongoing chronic disease management via remote patient monitoring technology. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating strong patient relationships, proactively monitoring patient vitals, conducting video visits, and providing care to keep patients healthier and remain out of the hospital. Graduate of an accredited school of professional nursing and a bachelor’s degree in nursing (BSN).
Current registered nurse licensure in the State of Ohio.
3 years professional RN clinical experience.
Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
A thorough knowledge of nursing theory and practice, including basic knowledge relating to nursing such as biological, physical, psychosocial, and medical sciences and their applications.
Demonstrated interpersonal skills to interact effectively with patients, coworkers, and other Medical staff in a virtual care setting.
Broad-based knowledge and clinical expertise in assessing, analyzing, intervening, and evaluating patient care.
Ability to take clinical on-call time.
Ability to prioritize and manage multiple tasks and function within an ever-changing environment.
Ability to concentrate for long periods of time, work well under stress, and be able to think and react quickly in emergencies.
Ability to coordinate and implement services for multiple patients with a variety of ages and diagnoses.
Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds.Preferred:
Previous telemedicine, Critical Care, or telemetry experience.
1 year of Home Health care experiencePhysical Demands:
May sit, stand, stoop, bend, and ambulate intermittently during the day.
May need to sit or stand for extended periods.
See in the normal visual range with or without correction.
Hear in the normal audio range with or without correction.
Finger dexterity to operate office equipment required.
Ability to communicate and connect with patients using virtual tools, telephonic and video face-to-face, email, and other communications.
Ability to see computer monitor and departmental documents.
