We care for your career while caring for the Paid Time Off and HolidaysProfessional AdvancementEmployee DiscountsSUMMARYThe Care Manager - Service Coordinator coordinates and implements the care management functions and services of Care at Home, including but not limited to private pay care management services and the service coordinator functions of the Nursing Home Transition Diversion Waivered Program (NHTDW) program.STATUS: Part Time (20 Hours)LOCATION: St. Mary's Campus - Rochester, NYDEPARTMENT: Nursing Home Transition Diversion Waivered Program - Care ManagementSCHEDULE: Weekdays - Flexible ShiftATTRIBUTESBachelors Degree in Social Work or a related degree required. Three to five years of experience in a Social Work setting which focused on elder care required.Evidence of familiarity with community resources. RESPONSIBILITIESCoordination. Coordinates and oversees care management functions to ensure quality, quantity, timeliness, and effectiveness of service to the NHTDW program clients.Assessment. Comprehensively assesses referred NHTDW and private pay clients personal and health related goals, as well as their physical, functional, psychological, social, environmental and financial status. Assesses informal and formal support systems, in addition to other situational factors which may impact on their level of risk and subsequent care management needs. Completes periodic reassessments and implements plan modifications based on clients progress or lack of. Completes all required documentation of oversight agencies, including NHTDW program, grant programs and other future funding sources as they occur.Care Planning & Development. Develops, implements, and revises plan of care through active communication with the client/family and the PCP, focusing on prevention and goal oriented approach and reduction of risk. Demonstrates current working knowledge of Medicare, Medicaid, HMOs, Blue Cross and Blue Shield, and Private Insurers. Community Resource & Education. Creatively researches and utilizes the full spectrum of community resources to meet the individual needs of each client. Educate the clients and families and facilitate access to needed services.Monitors, records and reports outcomes and variances i.e. key indicators, quality report, NHTDW service coordinator's reports and conferences as assigned. Facilitates and/or participates in ongoing NHTDW training/meetings, Community Based Service, and/or UHS committees or groups. EDUCATION: BS (Required)PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.LICENSES/CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)PAY RANGE: $55,500.00 - $60,000.00The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
