JEDC seeks someone with a deep understanding of eldercare resources in Southeast Alaska gained from several years of direct experience to help remove barriers to care for elders and their families. This work aligns with the mission of the Southeast Alaska Eldercare Coalition (SREC) which is to “Bring eldercare providers together to problem solve mutual issues, share program information, and be a collaborative force for change.” 

DUTIES

- Serve elders across Southeast Alaska by being the point person for all things eldercare across the region specific to navigation services.

- Participate in the development of a dynamic online tool for elders and navigators to assess needs, and provide resources that are easy to understand, utilize accessibility standards for elders, and serve as a one-stop shop for consolidated information.

- Monitor website and recommend any changes needed to reflect current practices.

- Provide information and referral to elders, family members, and other agencies.

- Identify resources for clients to overcome barriers to care.

- Provide outreach and support to rural communities through travel (as needed) as well as telephonically.

- Remain aware of current services offered by service providers within the region and statewide if needed.

- Maintain documentation of all client encounters and complete reporting requirements according to organization standards. 

- Able to build relationship building with different types of people, including clients, SREC Member Organizations, and health care providers.

- Excellent communication and interpersonal skills, as well as the ability to speak concisely to clients and family members.

- Ability to behave and communicate in a professional manner in the presence of clients, co-workers, supervisors, and external parties; write reportsincludewritten explanations, summaries, reports, and correspondence; and engage with community partners to ensure outreach materials are culturally relevant to Southeast Alaska.

- Commitment to maintaining membership in related professional organizations, staying current with any required certifications, and attend education exchanges and/or industry related meetings, and developing professional contacts to expand professional network.

- Attends seminars and meetings to stay abreast of changes in Alaska's healthcare system.

- Assist in the coordination of a large regional Eldercare Summit on October 1-2, 2024.

- Seek additional funding to ensure sustainability of program.

- As applicable, adhere to HIPAA regulations and standards of confidentiality.

- As needed, coordinate other related duties as assigned by the SREC ProgramDirector.

QUALIFICATIONS

- Must have 4-5 years' experience in a social services navigator role in Alaska; very strong preference for knowledge specific to Southeast.

- Must have demonstrated ability to coordinate calendars, meet deadlines, track progress, analyze data, identify areas for improvement, make recommendations, and work well with others.

- Must have experience living and/or working in Southeast Alaska; travel to villages and communities in the region will be a plus.

- Must have a professional demeanor, excellent communication skills, and strong organizational abilities.

- Must have full proficiency with basic Microsoft Office applications (Excel, Word, PowerPoint).

- Must be able to operate in a virtual environment via video conferencing and collaborative online workspaces.

- Must have a high school diploma is required; post-secondary education in social science or public administration preferred.

- Prior emotional exposure to issues of life, death, and dying preferred.

- Experience with or willingness to learn about different cultures; knowledge of indigenous values and culture is preferred.

- Experience planning large events will be a plus.

- Willingness to occasionally work after hours and on weekends, if needed.
