Polish Speaking Patient Care Coordinator

Provide monthly engagement to patients enrolled into Ceras Platform for Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration Program (BHI) and (DTCM) Digital Transitions of Care other services.

Clearly and articulately explain the value of Ceras Health services to patients/caregivers.

Provide through Electronic Medical Record (EMR) review, successfully extract pertinent data and enter the Ceras platform.

Ensure all diagnosis codes, previous patient visits and medications are accurate and confirmed during patient interview.

Schedule follow-up engagement service with assigned care coordinator on the care team.

Schedule provider appointments as required by CMS for service compliance.

Become subject matter expert for patient care management and care coordination activities.
