LPN (Licensed Practical Nurse)

Licensed Practical Nurses are a vital part of the success of our organization. We are seeking individuals with a passion for helping others and strong communication skills to join our team. The Licensed Practical Nurse will be responsible for communicating information to our providers and patients, executing orders as directed, and working closely with patients that have chronic kidney disease. The Licensed Practical Nurse must be willing to work in a clinic setting and have at least one year of previous experience.

Your responsibilities are:

-Provide nursing assistance during patient visits - rooming/education

-Take patient phone calls

-Relay information to the Physician or Nurse Practitioner

-Execute Orders as directed

-Keep exam rooms stocked and clean

-Pull records from hospitals and other practices

-Schedule patient visits

-Work as a patient care team

-Any additional tasks the Clinical Coordinator needs.

-Must be willing to travel to other clinics as needed. 25% of travel.
