Job Type: Full TimePay: $40,000 per yearRewarding Work with Remarkable People - Excellent Leadership Opportunity-Essential Role in the Person-Centered Service Plan ProcessThe Vocational Training Center (VTC) of Fargo, ND is now searching for a candidate to join our Program Services team as a Case Manager (QDDP).Responsibilities of this position include:

Developing, monitoring and reporting on support plans

Conducting assessments and creating progress reports

Coordinating and facilitating support network meetings

Traveling to local community sites to assess progress of supported individuals' personal objectives

Training Direct Support Professionals on methods, strategies and techniques relative to supported individuals' programs and active support

Minimum of Bachelor's degree in social work, human services or other related field.

Minimum one year experience working with individuals with intellectual and developmental disabilities, mental health or related field.

Must complete the competency-based modules as dictated by the North Dakota Department of Human Services, Developmental Disabilities Division.

Current driver's license

Strong interpersonal, oral, and written communication skills
