Valley View Village, a senior care campus in Des Moines, Iowa, is hiring a Full-Time Director of Social Servicesto join our team! The Director of Social Services will advocate for our residents, families, and staff and provide overall leadership and management. Our ideal candidate thrives in a fast-paced environment, excels in multitasking, and has excellent customer service skills. This is a Full-Time, benefit eligible position. Director of Social Services responsibilities:

Provideguidance and direction to the social services department through policy and procedure creation and monitoring to ensure the Cassia mission is being realized.

Coordinate caseloads, complete social history, care plans, MDS, and discharge plans for new residents.

Engage with community partnerships.

Maintain and supervise the budget.

Participatein resident care conferences involving the planning and implementation for medically related social services, mental healthservices, and emotional support services.

Oversee and complete referrals for resident mental health services and coordinate consultative services with providers.

Complete social servicesadmission evaluation, as well as discharge, care plans for residents.

Maintain necessary documentation in resident's medical records.

Perform other duties as assigned.

Director of Social Services skills and requirements:

Licensed Social Worker in Iowa.

Bachelor of Social Work (BSW) or Master of Social Work (MSW) degree.

Experience in Long Term Care (LTC) or transitional care setting preferred, but not required.

Previous supervisory experience preferred.

Prior experience in the admission process, discharge planning, or management preferred.

Strong knowledge of local, state, and federal guidelines related to long-term care and resident rights.

Strong leadership, written and verbal communication skills.

Good communication and the ability to multi-task.
