2 Full-Time and 1 Part-Time positions up for grabs

Hybrid role (because saving the world can sometimes be done in pajamas)

Compensation: $130,000 to $140,000 annually

Qualifications:

Required: BSN

Required: California Registered Nurse Practitioner (NP) License

5+ years of clinical experience

3+ years in care management experience in health plan, home health, or hospice

Your Daily Adventures/Job Description:

Develops condition-specific alert-driven care pathway templates, incorporating risk levels and member actions for each event type.

Utilizes Chronic Condition Care Pathway templates with alerts to create personalized Member Action Plans.

Implements Remote Patient Monitoring and encourages Patient Self-Reporting for high-risk chronic conditions.

Conducts in-home or telehealth assessments as needed.

Responds promptly to patient alerts, maintaining active communication via text and phone with members and caregivers.

Monitors remote patient monitoring tools, promptly notifying the interdisciplinary team of any out-of-range results or clinical outcomes that require follow-up.

Coordinates care for patients, including patient navigation, chronic disease management and education, and interdisciplinary collaboration, in adherence to department policies, procedures, and contractual requirements.

Collaborates with patients and their care team to conduct assessments that inform prioritized Member Action Plans, tracking outcomes, interventions, and reassessing needs as necessary. Establishes appropriate follow-up frequencies and ensures seamless referral services as patients progress through the care continuum.

Empowers patients to take proactive roles in managing their health, medications, treatments, mental health needs, and follow-up appointments, facilitating referrals to community-based organizations or other programs as needed.

Adheres to evidence-based guidelines and contact standards to close gaps in care, promote the use of in-network services, and determine when in-home services are warranted and ordered.

Utilizes electronic medical records or Care Management platforms for care coordination activities, ensuring compliance with workflow policies and consistent documentation.

Participates in team-based rounds to contribute to ongoing program design and development, integrating insights and lessons learned from field experiences and process improvements within the department.

Reassesses Member Action Plans post-discharge to ensure continuity of care.

Demonstrates flexibility in prioritizing patient care workload effectively.

Collaborates daily with interdisciplinary teams to discuss patient care planning and coordination.
