Employment Type:Full timeShift:Day ShiftDescription:The Director is accountable & responsible for providing goal directed leadership in the management and direction of Quality and Population Health management processes, projects, programs, and activities on behalf of the clinically integrated network (CIN). Demonstrates passion for achieving results and an uncompromising dedication to excellence in clinical outcomes. Implements major third-party payer Pay-for-Performance and quality improvement programs. Develops, implements, maintains, and continuously improves various internal and external medical management projects. Collaborates with Medical Group Leadership and independent affiliated providers to ensure consistency and integration of strategy & operations across Trinity Health.What the Director Quality Population Health CIN will need:

What the Director Quality Population Health CIN will need:

Bachelor's Degree in nursing, healthcare, business, or quality related programs.

Previous quality and population health leadership roles, with experience serving on quality committees. Seven years of progressively responsible management experience, including 7 years in Quality.

Master's Degree, preferred: Experience with Quality and Population Health management processes and tools.

Preferred, strong knowledge of clinical quality expectations of oversight of Healthcare Effectiveness Data and Information Set (HEDIS) guidelines, organizations such as National Committee for Quality Assurance (NCQA) and Centers for Medicare and Medicaid Services (CMS), as well as insurance companies such as Blue Cross Blue Shield Michigan (BCBSM), Priority Health and Blue Care Network.
