OVERVIEW: Under the supervision of the Clinical Program Manager, the Family Facilitator II coordinates the development and implementation of individualized service plans (utilizing High Fidelity Wraparound model) and provides direct services to children and their families in order to maximize the involvement of all persons and the implementation of plans, and facilitates the integration, coordination and implementation of services.

LOCATION: 2750 Sutterville Rd., Sacramento, CA 95820

SCHEDULE: Monday – Friday, 9:00 am – 5:30 pm

SALARY: $25.94 - $29.18 per hour, to start, PLUS up to $8,000 annual productivity bonus!

DUTIES:

Direct Services

Provide leadership in the assessment, development, implementation, and evaluation of individual wraparound plans.

Coordinate, schedule, and facilitate child and family team meetings (CFT) for development of individualized wraparound plans.

Coordinate services for youth based on their level of need identified by the CFT, including, but not limited to, referrals for psychiatric services, advocacy, therapy, etc.

Assume responsibility for the functional assessment, crisis planning, Strengths, Needs, and Cultural Discovery (SNCD) and implementation of services/coordination of resources for children and families.

Monitor and support the delivery of services by reassessing and revising individual plans as needed, ensuring that families have access to resources, and maintain effective relationships with child and family team members.

Provide crisis management to families and child/family teams through development of safety/crisis plans as well as administration of such plans.

Participate in the Wrap crisis on-call system.

Complete initial and annual clinical bundles using Avatar—this includes core assessment, client plan, CANS, CHQ, etc. Diagnosis and development of client plan shall be in collaboration with identified qualified Clinical Manager, LPHA or waived staff.

Write progress notes for each client/family session within required deadlines.

Complete all documents for the Wrap program, including but not limited to, tracking sheets, family safety plan, family review, family budget, assessment plans, etc., in a timely manner.

Assume responsibility for ongoing child and family team development.

Assist in the ongoing development and evaluation of the service program.

Serve as a role model and advocate for children and families involved in wraparound.

Monitor and ensure effective linkages with community and agency service providers.

Participate in training activities as directed.

Establish and maintain positive and effective working relationships with internal staff and external resources.

While a child is residing in the residential program, attend meetings regarding the child.

Adhere to policies, systems and standards of the Sacramento Children’s Home and all applicable programs, agencies and contracts.

Participate in supervision, staff meetings, process reviews and other assigned meetings.

Perform other duties as assigned.

QUALIFICATIONS: Must possess a Master’s Degree or PhD and 2 years of full-time/equivalent (FTE) direct care experience in a mental health setting, or

a Bachelor’s degree and 4 FTE direct care experience in a mental health setting.

One year of direct practice experience working with seriously disturbed children, adolescents, and families, and experience in engaging parents as partners and working successfully with families from diverse ethnic and cultural groups is preferred.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
