Description We are in search of a Case Manager to join our team in San Francisco, California. In this role, you will manage a diverse caseload of individuals, many of whom have come from a background of homelessness and may be dealing with mental health or substance abuse issues. This position offers a short term contract employment opportunity.Responsibilities: Develop and maintain strong rapport with clients to support their goals of improving life quality and retaining housing. Conduct initial and ongoing needs assessments to identify and address client needs. Regularly visit clients in their homes and provide support during monthly rent payments and unit inspections. Assist clients in dealing with unit habitability issues and liaise with hotel managers for pest control and unit inspection visits. Refer clients to job placement programs and other appropriate services as needed. Foster a supportive community among tenants by organizing regular community events and social activities. Provide support to hotel staff and clients on monthly check disbursement day. Maintain thorough documentation of all client interactions and services provided. Respond to client-related crises and provide de-escalation intervention when needed. Collaborate with other departments and external providers to advocate for clients' needs. Respond to all phone messages, client requests, staff requests, and emails in a timely and appropriate manner. Maintain confidentiality of client records and concerns. Attend all meetings as scheduled and participate in meetings as requested.
