Medicare Home Health agency in Broward County is looking for Per-diem Physical Therapist, Physical Therapist Assistant, Occupational Therapist, Occupational Therapist Assistant with Home Health experienced to give care to our veterans and elderly patients in their homes. (NO SHIFT WORK)

PT RATES ARE THE FOLLOWING BELOW:

PHYSICAL THERAPIST FOR A REGULAR VISIT: $60.00

PHYSICAL THERAPIST EVALUATION: $70.00

PHYSICAL THERAPIST RE-EVALUATION: $70.00

PHYSICAL THERAPIST DISCHARGE: $65.00

PTA RATE ARE THE FOLLOWING BELOW:

PHYSICAL THERAPIST ASSISTANT VISIT: $60.00

OT RATES ARE THE FOLLOWING BELOW:

OCCUPATIONAL THERAPIST FOR A REGULAR VISIT: $70.00

OCCUPATIONAL THERAPIST EVALUATION: $70.00

OCCUPATIONAL THERAPIST RE-EVALUATION: $70.00

OCCUPATIONAL THERAPIST DISCHARGE: $65.00

OTA RATES ARE THE FOLLOWING BELOW:

OCCUPATIONAL THERAPIST ASSISTANT VISIT: $60.00

QUALIFICATIONS:

1. Current Resume with Home Health Experience (6 months to a year experience is a must)

2. Current Florida License

3. Current Professional Liability Insurance (NSO Phone # 800-247-1500)

4. Current Florida Driver License

5. Current Social Security Card

7. Current Proof of Citizenship (USA Passport, Voter ID, Birth Certificate, Resident Card, Work Permit (WE E-VERIFY ALL DOCUMENTS)

8. Current CPR Card with signature on back or certificate with e-verification code (NO ONLINE COURSE ACCEPTED)

9. Current OR Old 4 Hour initial HIV/AIDS Certification

10. Current 1 Hour of HIV/AIDS update Certificate

11. Current 2 Hour of Alzheimer's Disease and Related Disorders Certificate

12. Current Medical Errors Certificate

13. Current Domestic Violence Certificate

14. Current OSHA Certificate

15. Current HIPPA Certificate

16. Current 2 hours of Infection Control Certificate

17. Current Physical Exam (NO MORE THAN SIX MONTHS FROM HIRED DATE)

18. PPD TEST/GOLD QUANTIFERON BLOOD TEST/CHEST XRAY (EMPLOYEES THAT ARE PPD POSITIVE BY HISTORY CANNOT HAVE ANY MORE PPD'S DONE OR X-RAY. THEY MUST ONLY BRING A GOLD QUANTIFERON BLOOD TEST RESULT. (MUST BE LESS THAN 6 MONTHS)

19. Current AHCA Background Screening Level 2 Finger printing (we will verify this)

20. Covid-19 Vaccination Card OR Exemption form will be provide in case no vaccinated.
