GENERAL PURPOSE:The social worker assigned to General Ambulatory functions as Team Leader for the service and primary therapist to individuals ages 18 and older who are severely and persistently mentally ill. As Team Leader, they provide supervision to an interdisciplinary group of clinical staff, as appropriate. The Team Leader may collaborate in policy and program development, operations managing, and plan program activities for the program area.The social worker is also an integral member of the Clinic, providing comprehensive mental health services for individuals who have serious and persistent mental illness. In this role, they work with clients to develop client-driven goals. Through participation on an interdisciplinary team, the social worker provides direct clinical service including individual, family and group therapy, crisis intervention, case management, and community outreach.RESPONSIBILITIES:

Performs screening, diagnostic formulation, evaluation, and psychosocial assessments of patients and their families. Provides social work and behavioral health treatment and education to patient and families. This may be in the following modalities; individual, family and/or group therapy. Coordinates activities related to the comprehensive treatment and discharge planning for the patients assigned to his/her caseload. Participates as a member of an interdisciplinary treatment team involving consultation, psychosocial diagnosis, treatment planning, and periodic care review.

Functions as Team Leader and may assist in providing clinical supervision and contribute to the evaluation of an interdisciplinary group of staff. Assists in onboarding activities for new therapists in the clinic including policies and procedures as well as clinical teaching/orientation. Provide student/internship supervision, as appropriate. Manages interdisciplinary treatment team meetings, to include facilitating discussion of clinical cases and documenting minutes of those meetings.

Collaborates with appropriate collateral contacts and provides linkages to community resources. Coordinates with other resources in the community and within SMH to ensure continuity of care for identified patients. Provides back-up as requested and able to other units/programs during the absence of certain co-workers.

Documents treatment in patient’s electronic medical record in compliance with Division, Department, and Hospital Policy.

Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Documents all interventions and contacts in patient's chart in compliance with Division, Department and Hospital policy. Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities.

Provides coverages and accessibility for intakes, vacations, sick calls, as needed. Other duties as assigned

Teaching: Based on experience and as determined by the social work supervisor, the social worker may supervise social work or other master’s level graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.

Research: Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.

QUALIFICATIONS:

Master’s degree from an accredited school of social work (CSWE) required.

3-4 years professional social work practice beyond the Master's degree, at least one of which must include psychiatric emergency, crisis or outreach work; or an equivalent combination of education and experience and working with adult populations preferred.

Prior experience in providing individual or group therapy in an acute clinical setting preferred.

Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions preferred;

Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families preferred;

Dedication to delivering a team-based integrated model of care preferred.

Demonstrates cultural humility and anti-racism values and works toward ensuring all patients feel welcome preferred.

Ability to provide clinical guidance and supervision preferred.

Licensure as a social worker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start required.
