Administrative/Care Coordinator - REMOTE

The Administrative/Care Coordinator will assist assigned teams, nurses or social workers with the coordination of health care activities for members. This position is fully remote but must be local to the Ventura, Los Angeles or Santa Barbara Counties.

Monday-Friday 8:00am-5:00pm

$18.00-$21.50/hour

Responsibilities:

Conduct telephone interviews with member, significant others and family members to determine if care needs are being met or if additional services are needed.

Direct members to available resources and educate patients on a wide range of programs and options available to them.

Review and process all referrals as well as communicate with providers, members, PCP or referring parties for services.

Create and process program referrals.

Schedule assessments; coordinate and obtain medical records, ensuring processes are completed within regulatory timelines.

Document all interventions, follow up information and communication with members in the members EMR.

Establish telephonic, electronic correspondence and/or fax contact with internal and external customers following internal policies and HIPAA guidelines.

Other duties as assigned.

Requirements:

HS Diploma or GED

Prior administrative, customer service or data entry experience

Bilingual in Spanish (fluent)
