Mental Health Assistant
GENERAL PURPOSE:Under the supervision of a licensed social work supervisor but with reasonable independence, the Alzheimer¿s and Dementia Care Program MHA provides consultation and resource linkages to Alzheimer¿s and Dementia Care Program patients and their families. Responsibilities include intake, outreach, supportive and educational case management services to patients and families in their home setting as well as within the ambulatory clinic, served by the Alzheimer¿s and Dementia Care Program and Memory Care Program. The MHA collaborates with the Memory Care Program team, most commonly the medical director and the practice NP¿s. The MHA also provides consultation to the health care providers regarding patients' care needs and/or community resources. The MHA will also perform a variety of clinical tasks in support of patient care including, but not limited to; rooming patients for clinic visits, collection of standardized testing data, documentation in eRecord.RESPONSIBILITIES:
Participates in Alzheimer¿s and Dementia Care Program/Memory Care meetings as indicated. Participates in supervision/consultation about his/her work in accordance with plan development with supervisor and practice site MD Directors. Participates in required activities of the Alzheimer¿s and Dementia Care/Memory Care including, but not limited to, timely completion of statistical reports/forms, time reporting and other Memory Care or Division forms; training programs and University Health Service reviews. Participates in assigned committees.
Knowledge of the UCLA ADC model. Participates in relevant training and educational opportunities designed to enhance knowledge and skills. Provides consultation to other care providers regarding patient problems and community services.
Provides social work services and/or consultation in collaboration with NPs for patients with problems identified through psychosocial assessments. Services may include: crisis intervention; problem-solving; information and referral, respite care planning; end of life support; and assistance with environmental, transportation and/or legal concerns. Services are largely provided within the patient/family home or by phone. Collaborates with other practice care providers in development and implementation of interdisciplinary care plans. Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients in collaboration with medical director and practice NPs. Provides educational support to patients and caregivers and maintains current knowledge of community resources relevant to the Memory Care patient population.
Rooming Patients: Escorts patients from the waiting area to exam areas. Ensures patient wait time is minimized. Completes pre-visit appointment information which may include but is not limited to obtaining vital signs, med reconciliation, completion of patient screens in eRecord, and performing diagnostic testing.
Collaboration with Interdisciplinary Team: Touches base daily with nursing team (tech/RN/LPN) to discuss the daily schedule and where support may be needed. Anticipates provider needs and adapts plan to the complexity of the patient. Accepts and acts on constructive feedback from leadership, peers, and colleagues in a positive manner.
Attend 1:1 meetings with nursing leadership quarterly and as needed.
Participates in regular supervision, both scheduled and on an as-needed basis.
The MHA is accountable administratively and clinically to his/her social work supervisor and adherence to policies, procedures and standards of professional conduct. The MHA¿s job performance must conform to the expectations set forth in the position description. In addition, the MHA is accountable to Alzheimer¿s and Dementia Care/Memory Care Program practice site Medical Director for providing social work services in accordance with policies and standards established by Alzheimer¿s and Dementia/Memory Care.

QUALIFICATIONS:
Bachelor¿s Degree in Social Work, Psychology, or other human services related degree required.
1-2 year¿s experience in a clinical or home care setting (some ambulatory care experience preferred).
Excellent communication and team skills including ability to form strong collaborative interdisciplinary partnerships across care settings. Sound computer knowledge and skills including an aptitude for using health information technology to guide care management activities and functions. Ability to work independently and meet deadlines. Creativity and strong organizational skills preferred.
Valid NYS driver¿s license required including access to reliable transportation that enables fulfillment of position¿s travel requirements.
