The Quality Data Abstractor is an employee of Dignity Health Management Services Organization (Dignity Health MSO).Position Summary:The Quality Data Abstractor supports the Clinical Performance department by abstracting clinical data from medical records (paper and electronic) to ensure compliance with quality measures like HEDIS. This role involves making outreach calls and texts to assist members in scheduling medical appointments, closing care gaps, conducting medical records reviews, and performing payer audits throughout the year. The position plays a key role in enhancing the organization's overall quality improvement initiatives.Responsibilities may include:

Gains clear understanding of criteria for compliance with ACN clinical quality measures (numerator and denominator).

Enters patient demographics into appropriate data collection tools. Ensures a high degree of accuracy of information entered.

From measures identified in data collection tool, reviews chart for applicable documentation to support quality measures. Gathers data from patient charts and record results via data collection tool.

Works with practice physicians and clinical staff to set abstraction appointments, pulls charts according to the protocol and arranges for all necessary tools to conduct abstractions.

Collaborates with Data Analytics team to ensure accurate reporting of abstracted information from quality measures.

Provides accurate and prompt post-assessment findings to the Clinical Performance Department for review and follow- up.

Identifies opportunities for the Clinical Performance Consultants to assist providers and staff with documentation, workflow, and process improvements to ensure clinical quality.

Aggregates summary data, tracks statistics, conducts analysis and completes standard reports.

QualificationsMinimum Qualifications:

High school diploma or equivalent required.

Knowledge of general office procedures and Microsoft Office applications. Excellent written and oral communication and interpersonal skills, required.

Computer proficiency as well as knowledge of database entry. Strong organizational skills, required.

Must be able to communicate clearly and concisely with all levels of individuals, sometimes in stressful situations.

Preferred Qualifications:

Minimum two (2) years experience charting in medical records and/or chart data abstracting experience, preferred.

HEDIS/GPRO experience strongly desired.
