NP/PA: Transitions of Care
OverviewSt. Luke's Health System is currently seeking a full-time Nurse Practitioner or Physician Assistant to join our Transitions of Care team supporting patients with virtual appointments from our Virtual Care Center after an acute hospital stay with the goal of safely transitioning patients from the acute setting to home. With this team, St. Luke's provides a unique and modern care to high-risk patients from their home, focusing on preventing readmissions and supporting at-home needs. Additionally, collaborating with care teams through in-person assessments and virtual care visits.The selected APP will help transition patients from an acute hospital setting to an outpatient setting, managing symptoms with remote patient monitoring equipment, telephone encounters, and virtual visits. This position does have a four day a week schedule, intermediate weekend coverage , with a required call rotation. This position requires someone experienced in internal medicine, medication management, needs to be comfortable with technology, has strong communication and collaboration skills and works well independently and with cross-functional teams.JOB REQUIREMENTS INCLUDE:
Graduation from an accredited school with a minimum of a Master’s Degree and certification, as required by the specialty, as well as eligibility to obtain licensure from Idaho to practice as a Physician Assistant.
Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.
Excellent communication skills to include oral and written comprehension/expression.
Ability to manage a fast-paced, busy clinical environment related to changing patient needs, including work with patients with acute, chronic, and complex disease processes.
Ability and willingness to exhibit behaviors consistent with standards for performance improvement and organizational values (e.g., efficiency & financial responsibility, safety, partnership & service, teamwork, compassion, integrity, and trust & respect).
