($18.00 - $22.00/hr., dependent upon experience) SHIFT: 8am - 4pm Monday - FridayBENEFITS:Weekly PaychecksTuition AssistanceReferral BonusesPeer MentorshipGenerous Paid Time OffJob Summary: Responsible for the coordination of all referrals made to the facility for admission, screening of applicant, facilitation of the actual transfer of resident into the facility from the transferring facility. Assisting the residents and family in the admission process and attempting to provide support that will enable a positive adjustment to placement or care for the resident and their family. Job Responsibilities:
Coordinates and processes all inquiries, requests for information, applications for admission, and referrals from other facilities
Gathers sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident
Prepares all admission forms, and documents and completes these with residents and/or family at the time of admission, along with distributing to all necessary departments as appropriate
Maintains a centralized 18 - month file detailing the date of receipt and disposition of applications
Interacts with discharge planners in an effective, efficient manner
Acts as a liaison between transferring institutions and the facility; establishes a good rapport with transferring institutions
Greets new admissions, escorts them to their rooms, attempts to orient to surroundings and staff as appropriate
Provides tours of the facility for applicants, their families or members of the community interested in learning more about the facility
Participates in marketing the facility and maintaining an acceptable census level
Refers facility and resident concerns to appropriate individuals
Reviews and/or revises the Admission Procedures and Policy Manual to the submitted to Best Practice for consideration
Maintains residents' rights
Maintains HIPPA privacy, security and confidentiality standards
Follows all established policies and procedures
Maintains Corporate Compliance standards
Prepares any other necessary duties as assigned by Facility Administrator
Job Qualifications:
High School diploma or equivalent
Minimum of 5 years of experience in Long Term Care Facility working in Business Office or Admissions Department
Possesses leadership ability and the willingness to work amicably with the interdisciplinary teams
Knowledge of computer platforms and health insurance including Medicare, Medicaid, and private insurance regulations
Possesses aggressive sales acumen, customer service, and communication skills
Ability to multitask in a fast-paced environment while adhering to company policies
Exhibits an attitude of helpfulness, team spirit and support to all team members
