Wherever we go we engage faith, relationship-building, and volunteers to lift-up and support Colorado’s most vulnerable citizens.PROGRAMThis position is for the Back Home SSVF (Supportive Services for Veteran Families) program which is a Rapid Rehousing program that utilizes a 90-day model to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. Primarily focusing on housing case management, but also offering wrap-around case management services assisting to connect veterans to Public Benefits, VA benefits, Health referrals, and Financial health referrals.ESSENTIAL DUTIES AND RESPONSIBILITIESThe Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination.
