Child and Family Team Facilitator, Bilingual
Wage band range $19.00 to $25.00 per hour; however, it is ultimately determined by contract obligations, the scope of the position, the candidate's relevant experience, and internal equity. Bilingual in Spanish, preferred and position is eligible to test for a bilingual pay incentive. This position will not be offered as a remote workstyle.
Under the direction of the Wraparound Program Manager and Program Coordinator Clinician, the CFT Facilitator functions as the Lead in scheduling and facilitating Child and Family Team meetings, while providing strength based, family centered and solution focused approaches that are aligned with the Integrated Core Practice Model and wraparound model of care. Child and Family Team meetings are to include the family, extended family, caregivers, community representatives, service providers and agency staff. The Child and Family Team Facilitator carries out the mission of Casa Pacifica as it relates to individualized needs of each family and Child and Family team. The CFT facilitator complies with Casa Pacifica’s mission, values, policies and best practice standards to protect children and preserve families.
DUTIES AND RESPONSIBILITIES (illustrated by typical activities):
Coordinate, schedule, and facilitate child and family team meetings using strength based, needs driven and solution focused approaches to develop an agreed upon action plan to support the family.
Upon receiving the initial referral will make reasonable efforts to contact all identified parities within 48 hours to schedule the team based meeting during a time the CWS or Probation Officer identified, that is convenient to the family’s schedule and location.
Orient all participants of the CFT to the purpose of the meeting and provide appropriate logistics prior to the scheduled CFT
Efficiently structure and facilitate CFT’s so that all areas are addressed and meetings last no longer than 2 hours
Integrate information from the Child Adolescent Needs and Strengths (CANS) measure into the CFT process
Promote child and family/professional partnerships and family decision making as a part of the CFT process.
Ensure that the voices of all participants are heard during the CFT process, and that the CFT draws on the family’s history.
Utilize Solution Focused Practice to build trusting relationship with the family while gathering detailed information about strengths and protective capacities of a family.
Use Solution focused Inquiry to use questions designed to strengthen an individuals or family’s capacity to achieve their goals by identifying past and present capacities, achievements, strengths, values and traditions and visions of their future.
Using the family history to provide input, develops a child and family plan that is strength based, needs driven and culturally relevant that will impact the placement decision made by CWS and the services provided to support the youth
Must be able to conduct the CFT in the preferred language of the family either by speaking the family’s preferred language or utilizing interpretation services.
Assist in providing crisis management to families and children when plans disrupt and on-site consultation to others involved in plans.
Maintain accurate documentation of all case related contact within 48 hours of the contact.
Submit required forms and reports in a timely basis.
Attend Case Review on time and arrived prepared to discuss critical issues in families engaged in the process. Additionally, participates in individual clinical supervision.
Establish and maintain collaborative and effective working relationships with both internal staff throughout all levels of the agency and with external resources.
Ensure plan created during CFT is distributed to all participants promptly following the meeting.
Provide written CFT summaries to the referring party within 48 hours following the CFT.
Participate in training activities as directed.
Assist in monitoring and ensuring effective linkages with community and agency service providers.
Maintain a flexible work schedule as necessary to provide support for families at varying times, days, and places, including early mornings and late evenings.
Maintain flexibility in providing services throughout Santa Barbara County.
Maintain Confidentially amongst all team members involved in the CFT process.
Participate as a part of the FURS hotline on-call rotation.
May perform other duties as assigned from time to time.
