And above all, it means exceptional care, simply delivered — and we couldn’t do it without you.Make a DifferenceThe Life Skills Clinician (LSC) monitors and provides care coordination and life skills training and development in collaboration with the care team to youth and/or adults with mental health and/or substance use disorders, including those with serious and persistent mental illness and chronic addiction.Exceptional Skills and QualificationsApplicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.

Care team: Collaborates as part of a multidisciplinary care team member. May assist with gathering data for comprehensive psychosocial assessments. Able to deescalate clients displaying aggressive behaviors. Documents services provided in Electronic Medical Record (EMR) based on policies and procedures. When necessary, will transport clients in personal vehicles.

Department Accountabilities: Complies with all applicable Behavioral Health Care Service policies and procedures. Actively involved in professional development and education, including mandatory training. Completes time cards, PTO requests, mileage reports, and other documentation per Community Behavioral Health policies and procedures.

Individuals may not perform all of these duties, or may perform additional, related duties not listed here.

Bachelor's Degree required in mental health or related field (Required)

Current Indiana Driver's License (Required)

Two (2) years of social service experience in a volunteer or paid role (Preferred)
