NEW GRADS WELCOME$60 AN HOURThe Long Term Care RN assumes responsibility andaccountability for the delivery of care for patients in a Long Term Care and/orAssisted Living Facilities. The nursedemonstrates ability to make clinical judgments in an effective and efficientmanner with supervision, as well as critical thinking and performance abilitiesin the coordination of patient care. Additionally, this individual performs according to nursing standardsand is accountable for managing patient care and assisting others in themanagement of patient care. Essential Duties and Responsibilities:
Utilizes the nursingprocess to assess, plan, implement and evaluate patient care in a Long TermCare/Assisted Living Facility
Performs focused interviews to identify specific needs
Assesses signs and symptoms indicating physiologic andpsychological changes in the patient’s condition
Collects, analyzes, and interprets data and information fromhealthcare team members and documents actual and/or potential nursing diagnoses
Documents the patient’s plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions as well as responses to interventions
Performs interventions according to identified priorities,plan of care, and the facility policies and patient outcome standard
Provides a safe, comfortable and therapeutic patientenvironment
Initiates and evaluates patient and/or family education
Uses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing priorities
Monitors safety and effectiveness of the environment andequipment
Participates in activities to facilitate good interpersonalcommunications with the chosen facility
Supports professional practice by observing the following:uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2)Standards of Nursing Practice; 3) Standards of Patient Care; 4) JointCommission, state and local laws; 5) Patient’s Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies
Performs other duties as assigned/necessary
Minimum Requirements:
Current Registered Nurse License for the state in which thenurse practices
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
