Health Home Care Team Leader
GENERAL PURPOSE:To provide supervision and oversight of Health Home Care Managers in the Strong Embedded Health Home Care Management Program. To collaborate with participating Practice Supervisors, UR enterprise and community stakeholders to ensure Care Managers follow the established parameters of the program, as well as workflows in accordance with NYS DOH regulatory guidelines and lead Health Home processes.Demonstrates ICARE values in each of the major responsibilities.RESPONSIBILITIES:Supervision of Care Managers for the Embedded Program:
Provides ongoing clinical and performance supervision for Health Home Care Managers assigned to their team
Monitors team and individual productivity, attendance, reviews and approves leave requests, adjusts caseloads to accommodate leaves of absences and addresses performance concerns in accordance with URMC guidelines
Provides performance feedback on a regular basis through individual and group meetings, annual performance reviews; develops performance improvement plans as necessary.
Coaching, consult and direction to the tea m and individual tea m members to ensure efficient delivery of timely, appropriate and effective services to the clients on their caseload.
Tracks and monitors that the services connected to the client are effective and delivered as planned.
Facilitate individual and group supervision to develop and refine care management and organization/documentation skills. Co-lead staff meetings and Health Home trainings.
Responsible for screening, hiring Care Managers with Director oversight
Auditing/Record Review/Compliance:
Oversees and com p l e t e s incident reporting for Care Managers assigned to their teams in compliance with the regulatory oversight agency (NYS DOH) and in collaboration with lead Health Homes.
Participates in the development and revision of organizational policies, procedures and workflows.
Monitors caseload sizes, providing accurate enrollment data to the Intake/Referral Coordinator, and coordinating/providing caseload coverage for members of their team when out of the office.
Responsible for regular auditing of records per prescribed workflows and on an as-needed basis.
Ensures Health Home care management documentation remains in compliance with regulatory guidelines, as well as URMC standards.
Engagement:
Reviews and discusses care plans with care managers and patients, focusing on linking individuals to clinical and social services with system and community providers. Monitors the use of costly Emergency and Inpatient services, supports patient in keeping aftercare appointments and addressing barriers as needed.
Health Home Supervisor will represent URMC by establishing and maintaining cooperative working relationships with community providers and stakeholders, both within and outside the UR enterprise, to obtain needed services and support for enrolled Health Home members.
Health Home Supervisors are responsible for conducting in-services in collaboration with the Assistant Director and Quality Assurance Specialist II on a variety of training topics including specific Health Home Care Management policies and procedures, community resources, etc.
Supervisors will foster a supportive, diverse and inclusive work environment
Assists Care Managers to utilize community and family resources in order to create sustainable support systems for patients, including with other programs and departments within URMC.
In addition to their supervisory responsibilities, Supervisors will provide Health Home Care Management services to Health Home members as part of coverage and support for their team, when necessary. Health Home Supervisors in collaboration with the Program Assistant Director are responsible for planning operations in terms of short- and long-term caseload coverage, and participating in decision-making that directly impacts program operations and policies.
Meet all required URMC and HH mandatory trainings, Health Updates, time reporting and other URMC staff requirements.
Other duties as assignedQUALIFICATIONS:
Bachelor's Degree in a related human services field required.
Supervisory experience required.
4 years of experience in providing direct services to people with serious mental illness, IDD, alcoholism/substance abuse, or experience effectively linking people with services that address social determinants of health or an equivalent combination of education and experience required.
Excellent verbal and written communication and interpersonal skills required.
Able to engage individuals and team, and hold care managers accountable for expectations of the program, lead Health Homes, URMC, and NYS guidelines required.
Must possess and maintain a valid New York State driver's license, have a satisfactory driving record and have access to an automobile. Must pass NYS DOH Health Home and URMC background check requirements.
