Supervisor Home Health Aides
Description Summary: In conjunction with the Director, Aide Services & Private Duty Agency, is responsible for the overall planning and supervision of the home health aides/home health aide program. In conjunction with the Director, conducts hiring, initial program orientation, and ongoing education of home health aide staff, makes home visits as necessary, monitors the quality of services provided to patients, assists in the referral taking process and interdisciplinary care planning. Assists the Director, with client visits and scheduling of companion/homemakers services and assists in coverage of the companion/homemaker service agency in the absence of the director.Responsibilities: 1.Under the direction of the Director, Aide Services & Private Duty Agency, the home health aide supervisor will participate in the review/revision of policies and procedures, hiring, program orienting, ongoing in-service education, and counseling of home health aide staff, working with the aide scheduler to assure appropriate coverage of aide services, and ensures that the patients� case managers provide home health supervision and orientation of patient aide services at individual patient homes. Makes home visits as needed. 30%2.Assists the Director with client visits and scheduling of companion/homemakers for Nuvance Caregivers at Home; assists in coverage of the companion/homemaker services during regular business hours and on-call coverage in the absence of the director. 30%3.Ensures compliance with licensing and regulatory standards. 5%4.Ensures compliance with the agency infection control, safety, and privacy policies. 5%5.Assists in the intake process of new patients, assuring appropriate coverage of aide services. 10%6.Ensures there is supervision of assigned home health personnel in the delivery of care on weekends and holidays. 10%7.Participates in quality improvement initiatives. 10%8.Fulfills all compliance responsibilities related to the position. 9.Maintains and Models Nuvance Health Values.

Education: CPN Other Information: Required: Minimum two years clinical experience, one of which is in the delivery of home health care; excellent organizational skills; flexible; experience with electronic medical records; excellent communication skills with a focus on conflict resolution and problem-solving; successful completion of nursing program; valid driver's license and vehicle in good-working order.Minimum Experience: three yearsDesired: Associate degree in nursing.Working Conditions: Manual: significant manual skills/motor coord & finger dexterityOccupational: Significant occupational riskPhysical Effort: Very Heavy effort.
